Biological characteristics of HBP:
Heparin-binding protein (Heparin-Binding Protein, or HBP)HBP is a protein secreted by mature neutrophils. Levels of HBP are elevated in cases of bacterial infection or acute respiratory distress syndrome (ARDS) or acute lung injury (ALI) and respiratory failure. HBP is not elevated in non-bacterial infections.
HBP is one of the earliest elevated markers of bacterial infection and can be detected in plasma within 1h after local or mild bacterial infection. This is an advantage over procalcitonin (PCT).
The level of HBP can also be applied to the identification of various types of post-surgical infections. [1
Structural characteristics of HBP
Heparin-binding protein is a neutrophil-derived granule protein. Its structure includes a single-chain protein of 222 amino acids, contains 8 cysteine residues, and has a glycosylation site at the 100th, 114 or 145 aspartic acid residue. The structure is similar to neutrophil elastin, and the homology is 45%.
Clinical significance of HBP:
In clinical, heparin binding protein can be used as a diagnostic index of local infection, auxiliary diagnosis of urinary tract infection, the prediction of septic shock and the prediction of microcirculatory failure in sepsis.
Comparison of HBP, PCT and CRP HBP: high sensitivity, strong specificity, high positive/negative detection rate, early infection can be detected, high concentration in acute bacterial infection, and low concentration in viral infection and non-specific inflammation. PCT: It can appear in the early stage of infection, which is increased when the body has a systemic response to infection, and the serum level is not significantly increased in local infection and chronic infection. CRP: A variety of infectious and non-infectious factors (surgery) can cause CRP to rise, which starts to rise 6-8h after infection, reaches the peak at 24-48h, and the specificity is not ideal.
